Plan Enrollment
Enrollment for the 401(k) plan is held twice each year on January 1st and July 1st but may be allowed more frequently at your employer’s discretion. To enroll or change your hourly 401(k)-deferral amount, you must provide a completed 401(k) Enrollment/Election form to your employer’s payroll department. Ceasing Deferrals
You may cease your 401(k) deferral at any time by notifying your employer and contacting the plan office listed above.
Changing Investment Elections
The provider of our 401k record keeping services is Milliman, Inc. You are able to access your account online or via a Voice Response Unit. The website for Investment Election Changes and Fund Re-allocations is: https://www.MillimanBenefits.com. If you prefer to call, the toll-free Voice Response Unit number is 1-866-665-5314. Both are available 24 hours a day, 7 days per week. Benefits Service Center Representatives are available Monday through Friday from 7 am to 7 pm (CST).
